Health Care Provider Information
It is church policy that "church-authorized imagery is generally limited to recording or photographing church events and activities and should involve group photos and/or individual photos that are part of a series of photos of an event."

PHOTO OPT-OUT
Church of the Redeemer takes care that pictures of minors not include indentifying information, such as their names. However, if you would like us to refrain from including your child's picture in church publications, such as website or email, please check below.
Off-Site Events (Permission & Authorization)
The following permission and authorization section pertains to activities and events taking place away from the church building (i.e., Kids' Listening Prayer, Bible studies, hangouts, service projects, field trips), in which children and youth are released into the care of church staff and/or volunteers and parents do not remain at the site of the event.
I give permission for my child(ren), named above, to attend events, field trips, and service projects associated with the children and/or youth ministries of Church of the Redeemer, St. Paul, MN.
For 6th–12th grade children:
I give permission for my child(ren) to be transported to and from events by hired and volunteer drivers authorized by Church of the Redeemer. Transportation will be arranged according to church policy.
Medical Release
I hereby authorize the Youth Group leaders, Kids of the Redeemer leaders, volunteers, Church of the Redeemer, hospitals, licensed medical or dental providers, and their agents and employees to have access to the information contained in this form and to provide all medical or dental care, routine tests, treatment, and necessary transportation advisable for the health and safety of my child(ren) listed above. This authorization includes the authority to consent to any x-ray examinations, anesthetic, medical procedure or treatment, and hospital care under the supervision, and upon the advice of or to be rendered by, a physician or surgeon licensed under the Medical Practice Act or dentist licensed under the Dental Practice Act for my child(ren).
Activity Release
I further give permission for my child(ren) to participate in all activities sponsored by the Children's and Youth Ministries of Church of the Redeemer, St. Paul, MN, except as noted below.
